Methylcobalamin vs Cyanocobalamin: Which B12 Form Is Better?

Methylcobalamin is a coenzyme form of B12 the body uses directly. Cyanocobalamin is a synthetic form that must be converted first; it is cheaper, more shelf-stable, and dominates mass-market supplements and fortified foods. What the two forms actually are
Vitamin B12 — cobalamin — is built around a cobalt atom. What hangs off that atom is what names the form:

- Adenosylcobalamin — the other active coenzyme form, working in the mitochondria. Sold less often, worth knowing exists.
- Cyanocobalamin — a cyanide group. Does not occur meaningfully in nature; it is a manufacturing artifact that turns out to be extremely stable, which is why it became the industry standard.
- Hydroxocobalamin — a hydroxyl group. The form typically used in B12 injections.
Your body converts whichever form you take into the coenzyme forms it needs. Cyanocobalamin has one extra step in that process; methylcobalamin arrives as a coenzyme form already.

Where the marketing overreaches
Three claims get repeated constantly. Each has a grain of truth and a stretch attached.
"Methylcobalamin is far better absorbed"
Both forms cross the gut through the same intrinsic factor pathway, and some research has found cyanocobalamin absorbed at least as efficiently — while methylcobalamin appears to be retained in tissue somewhat better. "Better absorbed" is not the accurate summary. "Retained differently, and already active" is closer.
"The cyanide in cyanocobalamin is harmful"
The amount released is trivial, the body neutralizes and excretes it, and cyanocobalamin has decades of safe use in supplements and fortified foods behind it. This is a talking point, not a safety concern for healthy adults.
"Some people can't convert cyanocobalamin"
Genuine conversion and methylation differences between individuals do exist, and this is the strongest version of the argument. But the practical significance for a healthy adult taking a daily supplement is not well established, and it does not justify the certainty with which it is usually asserted.
So does the form matter?
For most healthy adults, either form covers B12 needs. Where methylcobalamin earns its place is as a signal about the rest of the formula: it costs more, and a brand that pays for it on a line item most customers never inspect is usually making the same choice elsewhere on the label. The daily value for B12 on US supplement labels is 2.4 mcg for adults, so 375% DV works out to 9 mcg per serving. Labels are formulated well above the baseline requirement to account for it.
